Why should emphysematous cystitis be differentiated from emphysematous pyelonephritis?

They affect the same group of patients, but emphysematous cystitis is treated with antibiotics, whereas emphysematous pyelonephritis is a surgical emergency requiring percutaneous drainage or nephrectomy.
