Plain film and CT are insensitive for detection of nondisplaced fractures in osteopenic patients. If there is a high clinical suspicion and plain films are negative, MRI or bone scan may be obtained. MRI is the test of choice in the acute setting, as bone scan may be false-negative in the acute period or setting.
