In obstruction, there is a distinct transition point between the dilated segment and the normal segment.

In adynamic ileus, the entire bowel is dilated. In obstruction, dynamic air-fluid levels (i.e., air-fluid levels at different levels within the same loop of bowel) are present, which indicates peristalsis is occurring.
