Why does it matter whether the patient is upright or supine when the CXR is taken?
The appearance of air or fluid in the pleural space, the heart size, and the appearance of the pulmonary vessels are significantly affected by position. On an upright CXR, air will appear at the apex and fluid at the base; on a supine CXR, air will be anterior and fluid posterior.
