Why are some tumors cold on bone scan (no detectable radiotracer uptake)?

Some osteolytic lesions that are aggressive or rapidly growing destroy bone faster than it can be deposited by osteoblasts. Because a bone scan detects osteoblastic activity, these lesions are cold. (Multiple myeloma and renal cell carcinoma are associated with this phenomenon.)
