Describe the diaphragm sign

Fluid posterior to the diaphragm on a CT scan is pleural fluid, not ascites. Fluid within the dome of the diaphragm (centrally) is ascites. Because of the bare area of the liver, ascites cannot come between the liver and the diaphragm posteriorly.
