What are the indications for core needle biopsy?

Suspicious (BIRADS 4) or malignant(BIRADS 5) lesion; probably benign (BIRADS 3) lesion if the patient is excessively anxious about short-term follow-up, is unreliable, or has a medical condition requiring histologic diagnosis (e.g., transplant candidate or known breast cancer elsewhere in the same breast)
